Processing and issuance are subject to the submission of the following basic documents:

1. Notarized Affidavit of Undertaking

2. Dutiable taxes for the past three months

3. Valid ID of authorized signatory with three specimen signature

4. Updated GIS

5. PEZA Certification (for PEZA Locator) or Latest COR and Tax Exemption ( for Freeport Zone Locator)

6. Secretary’s Certificate with Board Resolution authorizing a person to sign the bond application.

7. Valid ID of corporate secretary with 3 specimen signature.
